I've noted this as a issue in the documentation comment for
WritableDatabase::replace_document(), so at least it will be documented
for 1.0.0.

Essentially this is due to the change (in 0.8.2) which allowed
replace_document() to add a new document.  Before that you'd have to add
more than 2 billion documents to a database to get this to happen.
