Is this database ever being moved or replaced?  The most likely reason
I can think of for this pattern of errors is that you're updating the
database by building a new copy, and then swapping it into place.  If
such a swap happened during a search, you could get an error like
this.

If you're only doing the updates by indexing directly to the database,
this can't be the problem.  If that's the situation, pehaps your
remote filesystem is reordering writes to the different files in the
database such that an inconsistent view is being presented to the
searchers.
