 ModifiedPostlist::skip_deletes doesn't skip over deletes when the deleted
 docid doesn't exist in the postlist. This can occur when a term is added
 and deleted in the same transaction. This affects both Flint and Chert.
 I've attached a patch (with testcase) that fixes the problem.
